and my family and I wound up living in a house that had been uninhabited for several years, 45 minutes from civilization, with the only source of heat being a Franklin wood-burning stove – upstairs. Ever try to push heat DOWNstairs? And a good part of every week was spent foraging for wood to keep my family warm.

So just don’t ask, OK?

As far as all the fun things that can go wrong with your Central Nervous System, spine, joints, etc., it’s a pretty long list. As a result, your doctor has an equally long list of tests to order for you. Now I am of the personal opinion that no doctor should ever order some of the tests I’ve had unless they’ve personally been on the receiving end of the test themselves. Seems fair, right?

I was diagnosed with Rheumatoid Arthritis in 1985 or so, but asymptomatic until around 2007.

I also had a lovely bout with tendonitis in my wrists for a few years in my 30s, for which my doctor and myself tried any number of different treatments, none of which was terribly successful. I simply outgrew the problem eventually.

Add to these, the long list of other spinal and skeletal issues, and you start to feel like a pincushion.

So let me give you a list of tests / procedures I’ve had – and you may be facing – and give you the honest lowdown on them – and my recommendations as a result. FYI: I’ve read a lot of misinformation about some of these tests on Internet forums. Consider the source. There is more MISinformation than INformation on the Internet.

I started studying electronics in 1960. I made my living in technology for most of my life. If I tell you something, you can bet I’ve had it done to me – up to 5″ long – and taps it into your skin directly above the muscle they want to test first. Then the tech taps on the top of the needle, causing it to penetrate further and further into the muscle, until the desired reading is obtained. Then – was originally engineered specifically for Tomás de Torquemada to extract confessions and sworn life-long allegiance during the Spanish Inquisition.

Here’s the lowdown. An electrical device with a pair of electrodes protruding from an insulated handle of sorts is connected through a flexible cable to a variable power supply. Electrode pads are stuck to the part of your body they want to test and cables are connected to a computer (they used an oscilloscope originally, but now everything is done on a computer). The technician dials up the appropriate amount of electricity for the beginning of the test, presses the electrodes into your skin where they want to start testing and presses a button, Zapping the hell out of you, whereupon, you yell “Ow!” or something similar. Your tech may repeat the same test more than once in the same place, but when they do, the current is generally increased with each subsequent “zap”. Now, you may feel a compelling desire to play Whack-A-Mole — or Deck-The-Tech after a few of these, but that’s probably not the most productive was to spend your afternoon (in handcuffs).

The reason for this test is to see how much pain you can tolerate.

Not really. It just seems that way.

The reason is to see how fast electrical impulses are communicated between various points of your body, which helps in the diagnosis of numerous nerve-related problems.

Myelogram

And even more fun. This is a really valuable test when you have problems with your spine and they need to pin down exactly where the problem is. I’ve had 2 of these for different reasons. While it’s not excruciatingly painful, you do feel a lot of pressure for what seems like forever (usually about 15 minutes.) I would tell my doctor if he wants to do a Myelogram on me, he’d better have an anesthesiologist on hand to give me a nice little Propofol nap while they do it.

Here’s the drill. You are placed on a large Radiology table that pivots and rotates. There’s an X-Ray machine above you. Depending on the area they need to evaluate, you may be tilted during the procedure.

Your spine is injected with contrast dye and the table is tilted to allow gravity to distribute this dye to the area of concern, Then they start taking pictures (smile pretty now) until they have everything they need. Good Radiologists will be very careful about positioning your head for cervical testing. If the dye leaks through from your spinal canal to your brain fluid, it can result in a very nasty headache I’m told.

DiscOGram

The Granddaddy of no damned fun at all! Most cases are not as severe as mine, so your mileage will definitely vary.

Here’s the lowdown on this very useful but incredibly painful test:

When your doctor has reason to believe a certain spinal intersection (disc) is causing your pain, they order discography to pin it down exactly. In my case, all 5 Lumbar discs were tested in one session. I’ve never been in so much pain in my life. I literally could not walk for 3 entire days afterward.

Here’s how it works.

The patient is placed face-down on a table under an X-Ray head.

An Anesthesiologist puts the patient to sleep.

Spinal needles (ever seen one of these?) are placed through the back and into the disc(s) to be tested. One on each side of each disk. That was a total of TEN needles for me. The Doctor attaches syringes with contrast dye onto each needle.

Then they wake up the patient.

And inject each disc one at a time, take images of it, and ask the patient what the pain is like and where. And how it compares to previous pain episodes.

In my case, rinse and repeat NINE TIMES.

Then the patient is put back to sleep, needles are removed and off to a CT Scan of the entire area tested.

When the patient wakes up, the anesthesia employed during this test has the merciful effect of selective amnesia, so you won’t remember everything they put you through while awake and answering questions. However… Pressurizing the discs with contrast has the effect of bringing back all the pain you’ve ever had at those intersections – all at once. Be prepared. It’s painful as hell.

MRI

Magnetic Resonance Imaging, or MRI is used to image the soft tissues in your body. This provides valuable information about your nerves, muscles and organs that could be found out in no other non-surgical manner.. For many people, having an MRI is frightening as hell. Primarily because there’s not a lot of room to maneuver inside the typical MRI.

If you’re claustrophobic, you may want to be pre-medicated. Talk with your doctor in advance. Most imaging facilities these days also have what they call “Open MRI” as an option for the claustrophobics among us. They are C-shaped rather than tubular. Investigate these issues if you do not like confined spaces.

I find MRI machines a great place to take a nap.

CT Scan

Computerized Tomography is similar to an MRI in terms of the way the machine looks and seems to operate. However, the CT scan produces a 3-D image of your body and is especially useful for finding problems in your bones and joints. It’s a more compact machine and should cause no complications for claustrophobics.

X-Rays

The good old-fashioned X-Ray is still one of the most valuable diagnostic tools in use today. Contrary to the belief by some very nervous people… The amount of radiation employed in today’s X-Rays is so low as to be completely insignificant. Multiple X-Rays in today’s world are of no concern, unlike the 1940s.

Procedures

Joint Draining

The first time I had this done, I thought I was going to pass out. I was 15 years old and had what was at that time called “water on the knee” or “housemaid’s knee”. Swell. That’s what a teenage boy wants to be diagnosed with… My right knee was about the size of an average cantaloupe and was almost immobile. So my Orthopedist whips out a 25 cc glass syringe (to give you an idea how long ago this was) and a monster # 10 needle.

Seeing this combination alone was enough to unhinge my knees, so It’s a good thing I was lying on an examination table…

When he jammed what felt like a 16 penny nail into my knee, I thought my head was going to explode. Then he proceeded to fill the syringe completely, disconnect it and empty the syringe, then reconnect the syringe to the needle and repeat – for a total of 4 times – or 125 cc of fluid drained from my knee. This, of course, felt like an eternity. And just when I thought we were done… Not so fast there, speedy. NOW we’re going to inject some cortisone to quiet your knee down a bit.

OMFG! The entire world turned vivid blue and started swirling and pulsing. A free “trip”?
